I do not have “skeletons in the closet” that may cause me or UKIP embarrassment if they were to surface during an election.

I fulfil all the criteria laid down in this application, law, the UKIP Constitution, Party Rulebook and Standing Orders to serve as a candidate for the UK Independence Party.

I shall notify the Party Chairman immediately upon being interviewed under caution (i.e. As a suspect in a criminal case) and upon becoming subject to any legal proceedings in this country or abroad during my time as a UKIP candidate/elected member.

I will abide by any decisions made by the NEC throughout my candidature.

I shall notify the Party Chairman immediately should any circumstances which affect this declaration arise during my time as a candidate for UKIP or when elected to public office.

Candidates will be required to organise their own campaigns and activists with support from the Central Party. Prospective Parliamentary Candidates will oversee Council Candidates within their seat and will report to Regional Committees and the National Campaign Manager.

I confirm that if I am a candidate, I will give full public support to any current campaigns.

I agree to UKIP officers holding, disseminating, and using information and data about me.

I agree to speak to all willing members of the press in relation to my candidature, regardless of any feelings I may have regarding any media organisation or journalist.

I have read and fully understood the Proscribed List (https://www.ukip.org/ee-proscribed-list) and have never been associated with any organisation listed on it.

If applicable, I agree to participate in an assessment authorised by the Party Chairman. I will abide by the decisions of said assessment and in case of any dispute, I accept that the decision of the NEC is final.
